We are looking to recruit to the post of community services manager, to replace the existing post holder who has been successful in securing an alternative post. The community services manager is an exciting leadership role in adult Health and Social Care designed around a placed based model of care. This post is based in and around the city of Exeter (including surrounding villages), forming one of the seven clusters in the Eastern Devon area.
We are looking for a dynamic, collaborative and compassionate individual who can to lead and further develop the existing successful delivery models for this cluster. This is a joint role working for both The Royal Devon and Exeter NHS FT and Devon County Council, with key relationships and partnerships across a place based system including primary care and the local voluntary sector.
You will have a credible track record of managing and leading teams, establishing and maintaining effective relationships and working in partnership in order to deliver safe and co-ordinated high quality care for individuals, their families and communities. You will have a drive for community health and wellbeing, focusing on adults of all ages (including those in older age or with physical and learning disabilities). You will be key to the delivery of an integrated multi-disciplinary community health and social care team overseeing and holding accountability for local resources and services delivered to the local population
