Come and join a Community Nursing Team where you can feel valued, where you can make a real difference to patients and their families, and where no two days are the same. We have such an opportunity in our Tiverton-based Community Nursing Cluster for a Band 5 Registered Nurse.

You will work to provide exceptional nursing care to patients, predominantly in their own homes, and our aim is to maintain patients in their preferred place of care, supporting them to maximise their independence and optimise their quality of life.

You will be part of a dedicated and dynamic team, offering care through a diverse range of interventions, including end of life care, wound management and holistic assessment and implementation of care. You will work closely with allied agencies, such as social services, our therapy colleagues and GPs and their staff. Multi-disciplinary team working is an important part of the modern nursing role, and, especially in a community setting, effective communication skills are required to ensure this is successful.

Candidates will have excellent assessment skills, and will be responsible for the development, implementation and evaluation of personalised care-plans in partnership with patients, their families and carers.

Clinical skills will be supported by high-quality in-house training packages, and there is also the opportunity for varied personal and professional development. We provide a regular staff supervision programme, together with support for annual appraisal, revalidation and personal development.

The successful applicant will have good I.T. skills, be comfortable using digital technology, be of a flexible and adaptable personality, and willing to embrace change.

The post-holder must hold a current full driving licence, and as the post involves predominantly working independently, the ability to make confident, evidence-based decisions is necessary
