Exmouth WEB Community Rehabilitation Team is anintegrated multidisciplinary team approach of rehabilitation will work within a designated Cluster providing integrated health and social care delivery, with the patient and their family and/or carers to achieve their personalised rehabilitation goals.

This is an exciting opportunity for a motivated Occupational therapist to join our cluster to provide Clinical Leadership to our intermediate care team at Exmouth Community Hopsital. This will involve working with community and inpatient teams, working closely with occupationaltherapists, physiotherapists, nurses andsupport workerswho work within our intermediate care service.

You will hold a varied caseload, often consisting of multi-pathology presentations, providing the opportunity to develop excellent community rehabilitation, communication and case management skills. The post holder will work with different medical conditions including stroke, falls, those with long term conditions, post operations, neurological conditions, and generally older frail people. Assessments are holistic and outcome focused with goals and rehabilitation plans jointly agreed with the individual, with an aim of maximising independence.

We actively encourage professional development and have a regular in-house training and a supervision support system in place. As a Clinical Lead you will also participate in the Cluster Management Team, helping to shape the developmentof services locally. Depending on the background and experience of the successfu candidate there would also be an opportunity to take on other responsibilities and projects.
