The Tiverton and Cullompton Physiotherapy Team is expanding, and we have an opportunity for an enthusiastic and dedicated Band 6 PT to join our dynamic and evolving team.
The role will involve working in the community with patients in their rehabilitation programme and advising on equipment that will help to achieve their rehabilitation goals. A combination of telephone and video calls as well as face to face visits will be arranged to support the patient’s needs. The caseload will be varied, and therefore applicants should have post graduate experience of working with patients with complex long term, neurological and orthopedic conditions, as well as confidence in manual and therapeutic handling.
The post holder will work on a rota basis with the Occupational Therapists in the team in the Urgent Community Response Team under the support and guidance of the Urgent Community Response Team Lead, providing urgent visits to risk assess patients in the community to avoid hospital admissions or facilitate hospital discharges. This work also requires sound moving and handling skills, good problem solving and prioritisation skills, and communicating with other professionals in the well-established multi-disciplinary health and social care teams.
The post holder will be responsible for the supervision and development of band 5 Physiotherapists, Assistant Practitioners and Rehabilitation Support Workers.
Excellent communication and IT skills are required to coordinate the smooth running of the service within our large integrated health and social care team, including the support of junior staff members and use of the new digital patient note system.
We are committed to the continuing professional development of all our team members and as such we have regular local in service training, as well as opportunities for external training.
The role will encompass supporting innovative and evolving service improvements to maintain a high quality service working closely with our community hospital and acute sector colleagues.
