This is an exciting opportunity to join a busy Acute NHS Foundation Trust as Therapy Assistant Practitioner in Health for Older People on the Trust’s main site, the Royal Devon & Exeter Hospital (Wonford).
The post holder will work closely with the Team Lead Occupational Therapist, ward multidisciplinary team and Discharge Co-ordinator to provide an effective interdisciplinary service with assessment, diagnosis, treatment and advice to patients and their carers. The caseload will include patients with a wide range of acute and chronic conditions, many having complex needs.
The post holder will assess and formulate treatment plans to assist with the rehabilitation and discharge of patients following admission to the ward. The post holder will have strong assessment skills and be able to support physical and cognitive recovery through functional rehabilitation activities.
All members of the team will be expected to work across all areas and be available for the shifts across 7 days, including weekends and bank holidays.The post holder will work flexibly in other areas if required.
Typical hours 08.15-16.30
