We are looking for an Associate Community Matron to join our dedicated and highly professional team of nurses within the Crediton, Moretonhampstead and Okehampton cluster, working in the Devon location, including part of the Dartmoor national trust. This is a great opportunity for introducing innovative approaches to providing healthcare to people in their own homes, preventing admissions to hospital and being a pivotal member of a health and social care multi-disciplinary team. The post also gives the opportunity for engaging with our partners in secondary care and facilitating patient discharge from the acute sector.
In the Associate Community Matron role the post holder will be expected to have studied at degree level and able to demonstrate long term conditions assessment and management skills, and be working towards developing these skills further and be expected to work towards attaining the V300 Non- Medical Prescribing Qualification. The post holder will have knowledge and experience in long term conditions management, and work within an integrated team including medical colleagues, other health professionals, Social Services, voluntary agencies, to proactively manage, co-ordinate and support patients with multiple complex long term conditions. The post holder will work collaboratively with other appropriate services to ensure patients are appropriately stepped up and stepped down from the community matron caseload, and help patients and their families by providing health promotion and advocating improved techniques in self-management of there conditions.
This post brings great scope for integrated working with the community nursing team supporting caseload management and be able to demonstrate strong clinical leadership skills and develop and enhance relationships with other community matron colleagues and a range of specialist practitioners, as well as external partners. The position will include driving to support patients within their own home and to maximise place based care.
