The 'Ways2Wellbeing' scheme involves empowering client self-assessment through coaching and referral/support into a variety of activities which are typically provided by voluntary and community sector organisations, but also back into employment. Examples include volunteering, arts activities, group learning, gardening, befriending, cookery, healthy eating advice and physical exercise. It is designed to support people with a wide range of social, emotional, health or practical needs. Those who could benefit from social prescribing schemes such as Ways 2 Wellbeing include people with mild or long-term mental health problems, vulnerable groups, people who are socially isolated, and those who frequently attend either primary or secondary health care.

  • Improve health and wellbeing by working with individuals to assess and jointly agree their needs, personal challenges and goals.

  • Using coaching techniques and a strength based approach, to empower, support, motivate and encourage individuals to take responsibility for their own health and wellbeing by helping them to access services in the community.

  • On occasions the role may involve taking clients, who need high levels of motivation or confidence , to groups for the first time.

  • To work with local voluntary services and businesses to identify and creatively fill gaps in provision.

  • To work with the individual to enable a de-medicalisation model of their condition management by enabling them to manage their health and well-being through access to community based activities or employment.
