We are looking for an enthusiastic and motivated Team Manager to take on a leadership role within our supportive and dynamic multidisciplinary team. We are an integrated health and social care team providing urgent proactive and reactivesupport to patients in their own homes, either to prevent unnecessary hospital admissions, or to support people home from hospital. This is initially a 12 month post to support the reorganisation of the current large single team into two teams reflecting Exeter's move from one to two clusters. This post is being created to support the development of the Outer Exeter (South & West Neighbourhoods).

The successful candidate will have experience of working clinically and leading in different settings, operational line management, providing professional supervision, change management (design and implementation) and service development. Extensive clinical experience within urgent response work within the community setting is essential. Personal qualities such as emotional resilience, 'can do' solution focused approach and remaining calm and collected during times of increased pressure an pace are required.

You will have the ability to work as a confident and autonomous senior practitioner. You will need high level clinical assesssment and problem solving skills to manage high risk and acutely ill patients in the community. You will need to demonstrate flexibility, excellent time management skills and excellent communication and interpersonal skills.

The role will involve clinical leadership and operational management, the ratio between these three elements will flex to meet service demands.

This is an exciting and fast paced team, where each day can present new challenges and learning opportunities. Working 5 days over a 7 day period.

The postholder will have regular contact with vulnerable people and as such this post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.
