STROKE SUPPORT TEAM ROYAL DEVON AND EXTER NHS Foundation Trust The Stroke Support (Early Supported Discharge) Team is based in the Royal Devon and ExeterHospital but works with stroke patients and carers in their own homes. It delivers intensive, high quality stroke skilled nursing interventions, rehabilitation and care for patients with stroke, on their discharge home. The team are dedicated and flexible and work in a very interdisciplinary way to achieve excellent results. We have a rare opportunity for someone with the right skills and enthusiasm to join this team. Hours: 37.5, over five days, however it may be possible to consider less hours.
