We have an exciting opportunity to expand our team of Rehabilitation Support Workers at Tiverton Community Hospital. These posts will allow us to further develop the enabling ethos of the hospital wards, in line with intermediate care principles, in order to support people to return home.

There is a combination of fixed term and permanent positions available. One of these roles will have a focus on developing and coordinating activities, relevant to individuals goals that are set on the ward. Please can you specify on your application whether you would be interested in taking this focus The role will be varied and will require you support each person you work with to be as independent as is possible, by adopting an enabling approach to rehabilitation and support. You will be working with therapists, nursing staff, social care staff and other members of the multidisciplinary team, to support rehabilitation programmes to enable patients to achieve their identified goals at this stage of their recovery. The post holder will work with people who have different medical conditions including stroke, falls, those with long term conditions, post operations, neurological conditions, and generally older frail people.

The post is open to people who have already completed training in a relevant field to Diploma level 3 or who would be interested in furthering their education to the Diploma 3 level. We actively encourage professional development and have a regular in-house training and a supervision support system in place. The role will include working shift patterns so that support can be provided throughout the day to support people to meet their identified goals, enabling them to meet their activities of daily living and return home. The successful candidate will need to have good IT and communication skills, and have the ability to work independently as well as part of the team.

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service.
