This is an exciting opportunity to join a busy Acute NHS Foundation Trust as a respiratory nurse specialist for a new Early Supported Discharge team.

The post-holder is responsible for providing leadership & clinical expertise supporting the development of the respiratory early supported discharge team covering the community areas of Woodbury, Exmouth, Budleigh (WEB) and Exeter.  

The post-holder will assess and treat patients with complex respiratory conditions including acute, chronic and life-limiting cases and lead the service in the delivery of seamless transition between the acute trust and the community. He/she will be flexible to support their nursing, physiotherapy and occupational colleagues in ensuring safe and timely discharges and the on-going management of these patients in their own environment.

This post will involve assessing, treating and providing a holistic management plan to patients in their own homes and referring them on to other specialties or for further rehab including pulmonary rehab as required.   This position is based at the Trust’s main site, the Royal Devon & Exeter Hospital (Wonford), within a specialist clinical area covered by the Medical Directorate supported by the acute physiotherapy service.   The post-holder is responsible for his/her own workload within the designated clinical area; for the assessment and treatment of patients on a day-to-day basis;management and supervision of the early supported discharge team.

Caseloads will be flexible, including both ward work and in the community and at times will involve the provision of service to other areas of the Trust. This will occur particularly when working on call and at weekends. The post holder must be able to travel.
