Following the successful pilot of the Trusted Assessor role we are delighted to be in the position to be able to extend the service with permanent funding for an additional 1.4 WTE staff to make a 0.8 wte and a 0.6 wte posts.

The post holder will be predominatly based at the Royal Devon and Exeter NHS Foundation Trust supporting effective and timely discharges to individulas who are requiring Nursing Home Placement at the point of discharge from hospital.

They will be responsible for providing Nursing Homes with an comprehensive assessment of need to enable the homes to identify whether they can meet the needs of the individual without the need for a further assessment. This will speed up the discharge process and reduce delayed transfers of care.

The post holder will work closley with the health team in the Single Point of Access and the Social Care team within the Complex Hospital Discharge Team. The post holder will be required to work during the week and on Saturdays on a rota basis. Our vision is to provide safe, high quality seamless services delivered with courtesy and respect. The postholder will have access to vulnerable people in the course of their normal duties and as such this post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.
